Here are a few reasons why:
You don't have updated OnlyFans data.
You are trying to add an existing fan list to OnlyFans.

If your account is not synchronized, you won’t be able to add a fan spend list.
In such a situation, you will see the following error message:
What you can do to update your data:
Log in to the app and launch your account, or sign in to the extension.
After that, your account should be synchronized, and you can try adding a fan spend list again.
You are trying to add an existing fan list to OnlyFans
If there is already a fan list with the same name in your OnlyFans account, you won’t be able to create a new list.
In such a situation, you will see the following error message:
